How to Order
Place an order in the SAM Medical online store. Please contact Customer Service via e-mail at firstname.lastname@example.org or by phone at 503-783-6934 if you have any questions or need assistance. Customers will receive an e-mail order confirmation as orders are successfully placed in our system.
As of this time, SAM Medical ships to US customers only. Shipping for all orders less than $50.00 within the US is a flat rate of $3.95 per order. Free shipping on orders greater than $50.00.
Order and Shipment Procedures
Orders are typically shipped within 2 business days after payment is processed. SAM Medical will attempt to process order cancellation requests prior to shipment if able. If a portion of a customer order is temporarily out-of-stock, this portion of the order will be shipped to the customer as soon as available at no additional cost to the customer.
Customers will be notified via e-mail when their order is shipped, along with contact information for tracking purposes. To report a possible missing package, contact us at email@example.com or (503) 783-6934, but please allow a reasonable amount of time (5 business days) to pass before reporting it to us.

We guarantee that all the items we sell will meet your personal satisfaction. If for any reason you are not fully satisfied with your purchase, please call us to receive a Return Merchandise Authorization number (RMA).
If for any reason you are not satisfied with your purchase, you may return the item to us within 30 days. An RMA number may be requested by emailing us at firstname.lastname@example.org or by phone at (503) 783-6934. You will be provided with an RMA number and instructions for sending the product(s) back to us. To obtain the RMA number, please provide us with the invoice number and reason for the return.
Once you receive a return authorization you may return unused items, along with all accessories, within 30 days for an exchange or refund. All items must be returned in original packaging and in resalable condition. Returned clothing items must be unworn.
SAM Medical will not accept returned merchandise without a return authorization (RMA) number. This number must be posted in a conspicuous place on the outside of the package. Merchandise must include a copy of your invoice or packing slip.
Original shipping and handling charges are non-refundable. Please allow up to 2 weeks for credit for returned items. Shipping for returned merchandise will be paid by SAM Medical, at their discretion. Special orders may not be returned.
